Student Forms

Important Notes:

1. Students admitted to a Master's or PhD program as of Fall, 2017 have access to myProgress.
2. Student who registered before this time should email us at ipn [at] mcgill.ca. 
3. If you require the signature of the Graduate Program Director, please upload the form to myProgress. The form will be signed and returned back to you. 
4. For forms that need to be uploaded onto myProgress, please read through the myProgress instructions first. 
5. Transfer or "fast-track" students are to use the Ph.D. Candidacy Exam forms. 
6. Ph.D. Rotation students must complete and submit the Ph.D. Rotation forms to ipn [at] mcgill.ca, in addition to the regular Ph.D. forms. 

M.Sc. Students

Initial Mentor Meeting	Initial Mentorship Form	Please send an email directly to your mentor to set up a date and time to meet with one another. This can happen by phone or zoom. Upload the signed completed for to myProgress under "the study plan milestone."
Selection of Advisory Committee	Advisory Selection Committee Form	The IPN requires all new students to form an Advisory Committee within the first semester of starting the program. Upload the signed completed form to myProgress
Advisory Committee Meetings	2024 GPS Progress Tracking Report	How to complete the GPS Progress Tracking Report:  Instructions - Progress Tracking Report Form The Director will sign digitally after you upload on myProgress
M.Sc. Thesis Proposal	M.Sc. Thesis Proposal Form	The IPN requires the student to complete this milestone at 20 months. Upload the signed completed form to myProgress
M.Sc. Thesis Seminar	Student Poster	The student poster should be sent to project.ipn [at] mcgill.ca for circulation in our weekly newsletter.
	MSc. Seminar Approval Form	Upload the signed completed form to myProgress
