Self-declaration form for McGill staff

Please follow these directions if you are an employee who is in self-isolation, or has received a positive diagnosis for COVID-19.

Who needs to self-isolate?

You need to self-isolate if:

  • You have travelled outside Canada and returned on or after March 12, 2020; OR
  • You have been in close contact with a person diagnosed with, or suspected of being infected by, COVID-19; OR
  • You have experienced any of the following symptoms: fever, cough, difficulty breathing;

What to do if you are required to be on campus but now need to self-isolate

The self-declaration form must be completed by employees who are required to be on campus to provide necessary activities and functions, but are unable to do so because they must self-isolate.

What to do if you are NOT required to be on campus and need to self-isolate

If you are working from home, you do NOT need to complete a self-declaration form, even if you are required to self-isolate.

What to do if you are required to be on campus, but have a weakened immune system, a chronic condition or are aged 70 years and older

Employees required to work on campus to provide necessary activities and functions, but who have a weakened immune system, chronic condition or are aged 70 years and older, should remain at home and must send an email to disability.hr [at] mcgill.ca to inform them of their situation. The Disability Management unit (HR) will contact them to document their condition.

What to do if you receive a positive test result for COVID-19

Any employee who has received a clinical diagnosis of COVID-19 must complete the confirmation of illness form and send it by email to disability.hr [at] mcgill.ca. The Disability Management unit (HR) will contact them to document their condition. This process applies to any employee who tests positive for COVID-19, whether or not they are required to be on campus.

How to complete a self-declaration form

Employees who have access to Minerva

Please use the self-declaration tool found in the Employee menu.

  1. Log into Minerva
  2. Click on the Employee tab in the top menu
  3. Click on the link called “COVID-19 (Coronavirus) employee self-assessment”

Employees who do not have access to Minerva

  1. Please use the self declaration form
  2. Once completed, please send the form to your immediate supervisor by email

In both cases, the supervisors will send the form to their Human Resources representative.


All information will be treated confidentially. Supervisors, Human Resources representatives and the Disability Management Unit (HR) must keep the information collected safely. Any personal information must not be disclosed unless as required by law, or with the employee’s express consent.

Self-declaration form for McGill students

If you have been diagnosed with Coronavirus (COVID-19) please use the self-reporting form for students found in Minerva.

  1. Log-in to Minerva
  2. Select the Student menu
  3. Select “Coronavirus (COVID-19) Self Reporting”

Information collected will be kept safely and confidentially: any personal information will not be disclosed unless as required by law or with the student’s express consent.
