At the end of the presentation, the audience will:

  1. Recognize the long-term outcomes associated with hospitalization with acute kidney injury
  2. Identify gap in care following acute kidney injury that can be targeted to improve long-terms outcomes
  3. Understand evidence-based approaches to risk stratification and how they can be used to guide follow-up care plans in the community
