For Patients:
New patients will only be provided an appointment if they are referred by another physician. If you are in possession of a consultation request from your doctor to see a vascular surgeon, please fax the consultation to one of the fax numbers above and you will be contacted for your appointment.
On the day of your initial consultation appointment, it is important that you bring with you copies of any ultrasound scans, CT scans, MRI scans, Doppler tests or other X-rays that have been performed for the vascular problem for which you have been referred. A list of your current medications is also extremely helpful.
