I understand that this is a form to make an appointment in the Student and Staff Dental Clinic staffed by McGill faculty members and professional dentists, and not the Undergraduate Teaching Dental Clinic that provides care at reduced cost delivered by dental students. I acknowledge that the Faculty of Dental Medicine and Oral Health Sciences will also collect some private information (e.g. name, email) for patient intake and appointments.
