Paying Participant Form (Non-McGill)
External participants may pay by credit card or cash only. A receipt of payment will be provided. Please take note that invoices will not be issued.
If you are an external participant, please complete the
Paying Participant Form and fax it to (514) 398-8047, ehs [at] mcgill.ca (email the PDF to EHS) or mail/bring it to:

Please note: Send the form at least 1 week prior to the course commencement date. Incomplete forms will result in a delay in registration. If you don't have access to a fax, scan the registration form without the credit card number (complete all other fields; and call us at 514-398-4563 to provide the credit card number).
