Our Faculty has established the present questionnaire, for convenience.
We appreciate everyone's collaboration in answering the following questions. A record will be sent to the supervisor indicated below.
Please note: this questionnaire is for individuals working on campus only. Affiliated research institutes have put in place their own specific guidelines and procedures.
If you begin to experience symptoms in the workplace, you should be isolated in a room and, if available, should wear a procedural (or surgical) mask, or cover your mouth and nose with a clean cloth (e.g. scarf). Then please call 1-877-644-4545 for further guidance regarding next steps.
