The specimen shows fibrous thickening of the three aortic valve leaflets associated with a small degree of cusp fusion (long arrow) and small vegetations (short arrows).

Comment
The thickening and fusion of the leaflets suggests previous rheumatic fever. The specimen descriptive card describes involvement of the "mural endocardium" (white arrows); however, it is not clear if these foci are related to the anterior mitral valve leaflet or the ventricular myocardium.
