Then, complete the following 3 documents :
• Observation internship agreement contract
• Confidentiality form
• Judicial records declaration form*

It is also strongly recommended that the candidate obtain liability insurance if you are an international student.
All documents must be submitted in a single email with the supervising physician and department head in CC to jgh.medical.education.ccomtl [at] ssss.gouv.qc.ca
Please note that clinical directorates request magnetic access cards for medical interns.
Once the FORMS has been completed and all the documents have been received, our medical education team will confirm the internship.
