MISSA Feedback Form

This form is a pilot project to create an additional feedback mechanism for MISSA Council. As always you can also contact MISSA Council members in person or via email.

While MISSA will not be able to implement all suggestions, it is important for us to receive your feedback to stay abreast of the needs of the student body and help make your time at SIS an enriching experience.

If making a suggestion which will need a solution, we'd love to hear your input. A general anonymity note: by automatic default, the IP address of the computer used to submit the form is recorded as part of the form submission details.
