Nomination Form

Reference Period:  Deadline to Submit:
January 1, 2024 to December 31, 2024 Deadline: March 31, 2025 at 5:00 PM

A complete nomination consists of the following:

  • Table of Contents;
  • Nomination form;
  • Nomination letter, explaining why nominee should be considered for the award (NOTE: Nominees may not self-nominate.);
  • Statement from nominee, consisting of a list and brief description identifying measures taken by nominee in respect of equity and/or community building;
  • Up to 4 additional letters of support (from students, former students, colleagues, Chairs, Directors, supervisors, etc.).

Submit all materials as a single PDF file to equity.awards [at] mcgill.ca.
