It can be filled out by the patient, or it can be used by clinician as part of the interviews. Family members, caregivers, or other observers may provide an additional perspective on how the patient is functioning.

Research and clinical settings

 

Health and disability levels of the general population as well as specific groups (such as those with mental and physical disabilities) have been assessed using the instrument. It can be used across cultures and is suitable for adult population. It can also be used to design interventions or assess the impact of health and health-related interventions on service users.

 

Two versions exist:

  • Short version (12-item): brief assessments of overall functioning. It takes 5 minutes to complete the measure.
  • Long version (36-item): assessment of 6 domain-specific functioning:
  • Cognition: understanding & communicating
  • Mobility– moving & getting around
  • Self-care– hygiene, dressing, eating & staying alone
  • Getting along– interacting with other people
  • Life activities– domestic responsibilities, leisure, work & school
  • Participation– joining in community activities.

Time to complete

Short version: 5 minutes

Long version: 20 minutes

Cost/license

Free. A person who wished to use the form need to submit an online registration form on WHO website.
